چکیده

I n the pre-dialysis stage, the nutritional management of diabetic patients with chronic renal fail ure involves mainly caloric and protein intake and control of blood sugar. These aspects are very important because severity of hyperglycemia is correlated with the rate of progression of chronic renal failure, while strict glycemic control can markedly diminish the likelihood of nephropathy and retinopathy in patients with insulindependent diabetes (IDDM, type l) (1). Good glycemic control before starting dialysis has been shown to be an independent predictor of survival in patients with non insulin-dependent diabetes (NIDDM, type II) on continuous ambulatory peritoneal dialysis (CAPD); on the other hand, patients with poor glycemic control before dialysis have increased morbidity and shortened survival (2). Regarding protein restriction that may delay the progression of renal dis ease (3), one should avoid overzealous restriction, which would lead to protein malnutrition that may negatively affect outcome during subsequent dialysis. Adequate protein intake is important in end-stage renal disease (ESRD) in diabetic patients treated with either chronic hemodialysis or peritoneal dialysis, because mortality increases in a dose-dependent fashion with the degree of protein malnutrition. Protein-caloric malnutrition is a major complication of CAPD. An international cross-sectional study of 224 CAPD patients showed that 33% had mild malnutrition and 8% had severe malnutrition (4). In this study, female patients and diabetic patients were malnourished more frequently than male patients and non diabetic patients. Old age, long duration of CAPD, and minimal or absent residual renal function were factors predisposing to severe malnutrition.

Relationship between trajectories of serum albumin levels and technique failure according to diabetic status in peritoneal dialysis patients: A joint modeling approach

BACKGROUND In peritoneal dialysis, technique failure is an important metric to be considered. This study was performed in order to identify the relationship between trajectories of serum albumin levels and peritoneal dialysis technique failure on end-stage renal disease patients according to diabetic status. Furthermore, this study was performed to reveal predictors of serum albumin and techniq...

Correlation between Ultrafiltration Coefficient and Effective Lymphatic Absorption Rate in Continuous Ambulatory Peritoneal Dialysis Patients: A Possible Paradigm Shift

Background: The relative contribution of transcapillary water movement and lymphatic reabsorption in peritoneal dialysis (PD) is a critical issue, particularly in patients with ultrafiltration failure (UFF). Based on routine results obtained from the PD Adequest 2.0 software, the present study aimed to re-evaluate the separate effects of transcapillary water movement and lymphatic reabsorption ...

ارایه مدلی جهت پیش‌بینی بقای بیماران دیالیز صفاقی با استفاده از الگوریتم‌های داده‌کاوی

Background: Peritoneal dialysis is one of the most commonly used treatment methods for the patients with end stage renal failure. In recent years, the mortality rate of patients under this treatment has decreased; however, long-term survival is still an important challenge for health systems. The present study aimed to predict the survival of continuous ambulatory peritoneal dialysis patients. ...

Pharmacokinetics of gentamicin in patients undergoing continuous ambulatory peritoneal dialysis.

Teh pharmacokinetics of gentamicin were studied in seven chronic renal failure patients undergoing continuous ambulatory peritoneal dialysis. Patients received 100 mg of gentamicin in 2 liters of dialysate during an initial pass, and serum and dialysate samples were collected for gentamicin determination. Approximately 49% of the amount introduced into the peritoneal cavity was absorbed systemi...
